OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crush-by and struck-by hazards while lifting loads with underhung cranes and overhead hoists: a) Narrow Web Room (Cyco Slitter): Employees lifted spools of paper weighing about three-hundred (300) pounds using the 1-ton Dayton underhung crane and overhead hoist that have not been subjected to frequent and periodic inspections within the last twelve months. Employees were exposed to crush-by and struck-by hazards, on or about October 25, 2016. b) Wide Web Room (1618 Machine): Employees lifted spools of paper weighing about sixty (60) pounds using the five-hundred twenty-five (525) pounds Harrington overhead hoist and crane that have not been subjected to frequent and periodic inspections within the last twelve months. Employees were exposed to crush-by and struck-by hazards, on or about October 25, 2016. c) Wide Web Room (1007 Machine): Employees lifted spools of paper weighing about sixty (60) pounds using the five-hundred (500) pounds crane and hoist that have not been subjected to frequent and periodic inspections within the last twelve months. Employees were exposed to crush-by and struck-by hazards, on or about October 25, 2016. ABATEMENT CERTIFICATION IS REQUIRED FOR THIS VIOLATION.

ABATEMENT CERTIFICATION IS REQUIRED.

29 CFR 1910.1200(h)(3)(iv): Employee training did not include the details of the hazard communication program developed by the employer, including an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employees can obtain and use the appropriate hazard information: a) At the facility: Employees worked with hazardous chemicals, including, but not limited to chemicals found in inks such as UV Overprint Coating , and the employer did not provide these employees with the required training on the new label elements and safety data sheet format as required by the revised OSHA Hazard Communication Standard incorporating the Globally Harmonized System of Classification and Labeling of Chemicals prior to the effective date of December 1, 2013, as observed on or about October 25, 2016. ABATEMENT CERTIFICATION IS REQUIRED FOR THIS VIOLATION.
