29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity: (a) On or about October 12, 2016, HKK Enterprises, Inc. did not ensure that scaffolds and scaffold components were inspected for visible defects by a competent person before each work shift. A frame scaffold in use by employees was not inspected for visible defects before each shift. Scaffold did not have properly installed cross braces. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.452(c)(2): Frames and panels were not braced by cross, horizontal, or diagonal braces, or combination thereof, which would secure vertical members together laterally, and the cross braces were not of such length as would automatically square: (a) On or October 12, 2016, and at times prior, HKK Enterprises, Inc. did not ensure that frame scaffolds were secured with cross braces exposing employees to injury from scaffold collapse.
