29 CFR 1926.952(a)(2): A job briefing that meets paragraphs (b), (c), and (d) of this section was not conducted with the employees involved before they start each job. (a) Mountain Parks Electric, Inc. at mile marker 194 on Highway 40, Parshall, CO 80468: On and before October 19, 2016, the employer did not ensure that the employee in charge conducted a job briefing that met paragraphs (c)(2), and (c)(4) of this section with the employees involved before they started the job. This condition exposed the employees to electric shock hazards of 14.4 kV, phase to ground, resulting in an employee receiving a shock that was caused by lack of work procedures, special precautions, energy-source control, and personal protective equipment.

29 CFR 1926.964(a)(3)(ii): When a pole is set, moved, or removed near an exposed energized overhead conductor, the employee involved did not wear electrical protective equipment or use insulated devices when handling the mule tapes on the pole and the employee made contact with uninsulated parts of his body. (a) Mountain Parks Electric, Inc. at mile marker 194 on Highway 40, Parshall, CO 80468: On and before October 19, 2016, the employer did not ensure that each employee wore electrical protective equipment or use insulated devises when handling mule tape on a new pole being set near exposed energized overhead conductors. This condition exposed the employees to electric shock hazards of 14.4 kV, phase to ground, resulting in an employee receiving a shock that was caused by lack utilizing required electrical protective equipment.

In 2016, and at least the two prior years, all company injuries and/or illnesses from these three facilities were recorded on the Granby site OSHA 300 Log only.
