29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury.  a) Building 330, Langley AFB, VA 23665: On or about October 27, 2016, and times prior thereto, each employee exposed to hazards on an active construction site such as but not limited to falls while working on a mezzanine with an approximately 12'2" fall distance to lower level was not trained in fall hazard recognition and the procedures to be followed to minimize such hazards.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent:  a) Building 330, Langley AFB, VA 23665: On or about October 27, 2016, the employer did not record the workplace injury on the OSHA 300 Log or equivalent for the calendar year 2016 where a worker suffered a fracture of the left calcaneus in a fall that resulted in surgery and days away.  Documentation is required for this item.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours.   a) Building 330, Langley AFB, VA 23665: On October 27, 2016, at approximately 9:06 a.m., a worker suffered a fracture of the left calcaneus in a fall that resulted in an in-patient hospitalization. The employer did not report the in-patient hospitalization to the Occupational Safety & Health Administration. OSHA learned of the in-patient hospitalization on November 1, 2016.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
