29 CFR 1910.147(d)(2): An orderly shutdown of machine or equipment was not utilized to avoid any additional or increased hazards to employees as a result of equipment stoppage: a. On or about October 17, 2016, the employer did not ensure that employees utilized energy control procedures to control hazardous energy prior to cleaning scale out of the piping in the TMP chemical process unit. An employee was burned on the neck, back and arms after hot water was drained onto the hot coils of vessel E-810, creating steam, pushing the hot water out the open pipe onto the employee.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: a.) On or about October 17, 2016, the employer did not ensure employees were trained in the purpose and function of the energy control program. Employees were required to clean out scale in the E-740 to RT pipe to prepare for welding, and were not trained in proper lock out locations, resulting in an employee receiving burns to the neck, back, and arms from steam.
