29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: a) On or about October 11, 2016: Two employees unstrapped and were on the process of transferring a plotter from a pallet used as a working platform located on top of the forks of a Power Industrial Truck at a height of 10-feet from lower level without having a standard railing or any other type of fall protection, causing one of the employees to fall and suffered injuries to his back.
