29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about 21 October 2016, at the above addressed jobsite, three employees were working from the roof of an existing home, without the use of fall protection, exposing the employees to a fall hazard of approximately 9 feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about 21 October 2016, at the above addressed jobsite, three employees were working from the roof of an existing home, without the employer providing fall protection training, exposing the employees to a fall hazard of approximately 9 feet.

29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours:  On or about 21 October, 2016, at the above addressed jobsite, an accident occurred at the work site which resulted in a hospitalization of an employee. The accident was not reported to the Fort Lauderdale Area Office of the Occupational Safety Health Administration within eight (24) hours of the incident.
