29 CFR 1910.303(b)(2): Listed or labeled equipment was not installed and used in accordance with any instructions included in the listing or labeling:    a) Facility, where electrical equipment not used as listed or labeled included:  1. Office P-31, where a relocatable power tap was daisy chained with an extension cord and used to supply power to appliances including the water kettle. (Item Corrected)  2. Office P-35, where a relocatable power tap was used to supply power to appliances including two Seville portable fans.    3. Office P-18, where two relocatable power taps were daisy chained and used to supply power to the computer equipment. (Item Corrected)   4. IT Cubicle, where the desk top outlet, the uninterrupted power source (UPS) APC relocatable power tap and the Prime multi-outlet strip were daisy chained and used to supply power to computer and electronic equipment.  The Food and Drug Administration was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(b)(2), which was contained in OSHA inspection number 190241, citation no. 2, item no. 1, and was affirmed as a final order on March 21, 2012 with respect to a workplace at Bothell, WA.

1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy:   a) Rainier Conference Room, where the outlet used to supply power to the audio visual equipment was not equipped with a faceplate.
