29 CFR 1926.150(c)(1)(i): A fire extinguisher, rated not less than 2A, was not provided for each 3,000 square feet of the protected building area, or major fraction thereof. On or about 10/27/16 at the 3rd floor NW corner at stairway; the employer allowed employees to work in an area of 7000 SF where only one #10 ABC fire extinguisher was provided; thus exposing employees to insufficient fire protection means.

29 CFR 1926.501(b)(15): Each employee on a walking/working surface 6 feet (1.8 meters) or more above lower levels was not protected from falling by a guardrail system, safety net system, or personal fall arrest system: On or about 10/27/16 at the 3rd floor NW corner inside of stairway; the employer allowed employees to walk on the stairway platform where a 1/2" top cable was anchored to the wall 57" above the floor and the lower cable was anchored to the wall 37" above floor (with a 5" deep slack in it); thus exposing employees to a fall distance of 14' to the 2nd floor level below.
