A. High Ridge Construction Inc. worksite located at 27650 Lemoyne Rd., Millbury, Ohio: On or about November 2, 2016, the employer did not ensure the 12ft x 4ft working platform was securely attached to the forks of the rough terrain lift. The unsecured platform was at a height of approximately 14.00 feet while two employees were installing plywood sheeting.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level. A. High Ridge Construction Inc. worksite located at 27650 Lemoyne Rd., Millbury, Ohio: On or about November 2, 2016, the employer did not ensure employees were protected from a fall while working on the platform suspended by the rough terrain lift. The left side of the platform did not have functioning guardrail exposing the two employees to a fall distance of approximately 14.00 feet.
