Job Site - On or about 11-2-16 - The two by twelve inch planking used as a walkway to enter and exit the doorways of the building were not as least 18 inches wide exposing employees to fall hazards of 17 to 19 inches.

29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, was not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system. a) 911 New London Tnpk. Job Site - On or about 11-2-16 - Employees were working on the second floor of the building framing rooms and windows. The top rail of the guardrail system was at 51 inches above the floor in the window openings exposing them to 16 to 20 foot fall hazard.
