29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: On November 3, 2016, at the above addressed site, an employee was exposed to a fall hazard of approximately 14 feet, 6 inches when performing palm tree trimming work from the basket of an aerial lift while not wearing a safety belt and/or safety harness with a lanyard attached to the boom or basket of the lift.

29 CFR 1910.67(c)(2)(ii): An employee(s) operating an aerial lift(s) was not properly trained: On November 3, 2016, at the above addressed site, an employee exposed to a fall hazard when performing palm tree trimming work form the basket of an aerial lift that was not provided with a training program that would enable him to recognize the hazards associated with working from the aerial lift and falls. PLEASE NOTE: CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
