29 CFR 1926.20(b)(1): A safety and health program was not initiated and maintained to provide compliance with the general safety and health provisions of the standard: At the jobsite located at 740 N. 65th Street, Kansas City, KS 66102: Employees engaged in sewer system installation activities were exposed to struck-by and caught-between hazards in that a comprehensive safety and health program was not developed and implemented at this job site. 29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury: At the jobsite located at 740 N. 65th Street, Kansas City, KS 66102: Employees engaged in sewer system installation activities were exposed to struck-by and caught-between hazards in that the employer did not provide hazard recognition training for employees performing activities including, but not limited to, operating the Komatsu PC 240 Track; Caterpillar 963 Bob Cat; Hyundai Trackco; and the Takeuchi Skid Loader. Equipment operators were not certified prior to operating heavy equipment. 29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.
