29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:    1) Production Area: On or about November 2, 2016, the employer did not ensure the Mitsubishi 25 Pole Hyster model number FGC25 and serial number AF828-50621 was equipped with proper personal protective equipment such as a seat belt (operating restraint system). Employees were exposed to a struck-by hazard.    2) Production Area: On or about November 2, 2016, the employer did not ensure the Mitsubishi forklift truck model number FGC25B and serial number AF82B05303 was equipped with proper personal protective equipment such as a seat belt (operating restraint system). Employees were exposed to a struck-by hazard.

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s)    1) Production Area: On or about November 2, 2016, the employees were operating a Mitsubsihi 25 Pole Hyster (model number FGC25 and serial number AF828-50621) with defective and worn tires.  Employees were exposed to a tip-over hazard from operating it.    2) Production Area: On or about November 2, 2016, the employees were operating a Mitsubsihi Hyster (model number FGC25B and serial number AF82B05303) with defective and worn tires.  Employees were exposed to a tip-over hazard from operating it.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:    1) Production Area: On or about November 2, 2016, equipment such as boxes, paint bucket and other materials were stored in front of the 480 volts electrical panel box, creating insufficient working space for authorized personnel who were required to perform service or maintenance on the equipment.  The employees were exposed to electrical shock hazards.

29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy:     Production Area: On or about November 2, 2016, the electrical outlet box was not completely enclosed with covers designed by the manufacturer, exposing employees to electrical shock hazards of 110 volts.  The electrical outlet was missing a faceplate and supplying power to the lights in the building.
