29 CFR 1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:    (a) Unit 300 Phenol cake wash filter area: On October 30, 2016, employees engaged in filter changes (3259S, 3359S) were potentially exposed to Phenol due to not wearing the required flame retardant full chemical suit (jacket, pants, protective hood and chemical rubber boots).

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:    (a) Unit 300 Phenol cake wash filter area: On October 30, 2016, employees engaged in filter changes (3259S, 3359S) were potentially exposed to Phenol due to not wearing chemical goggles.

29 CFR 1910.132(f)(1)(ii): The employer did not provide training to each employee who is required by this section to use PPE on knowing what PPE is necessary:  (a) Unit 300 Phenol cake wash filter area.  Two chemical operators in training where not provided training on all the PPE that was necessary when changing filter media on filter 3259S and filter 3359S.  The operators were not aware of the requirement to wear flame retardant full chemical suite (Jacket and Pants), Protective Hood and Chemical Rubber Boots during the filter media change process.

29 CFR 1910.147(c)(6)(i): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed:  (a) Unit 300 Phenol cake wash filter area.  On October 30, 2016 one chemical operator trainee was changing filter media on filter 3259S when he was supposed to be changing the media on filter 3259 and he nor other employees were wearing the PPE required in the energy control filter change procedure.  The employer performs inspections of the energy control procedure on a 35 month schedule.

On October 30, 2016 one chemical operator trainee was changing filter media on filter 3259S when he was supposed to be changing the media on filter 3259 and an authorized person had not place a lock on the lock out box.
