29 CFR 1926.453(b)(2)(iv): Employee(s) working in an aerial lift were sitting or climbing on the edge of the basket:  a) 1 Park Place West. The employer did not ensure that employees working in an aerial lift were not sitting or climbing on the edge of the basket. An employee used an aerial lift to access the roof of a building that was under demolition, climbing in and out of the basket to access the roof, on or about November 2, 2016. Additionally, the employee was not wearing fall protection.   There is no abatement documentation or certification required for this item.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated  a) 1 Park Place West, Wood Ridge, NJ. The employer did not ensure that an employee working in an aerial lift wore a personal fall arrest system with a lanyard that was attached to the boom or basket when working from the aerial lift. An employee was observed using an aerial lift to access a high roof, without the use of a personal fall arrest system, on or about November 2, 2016.  There is no abatement documentation or certification required for this item.

The employer did not ensure that a tensioner, part of the structure of the building, was able to support at least 5,000 pounds per employee attached: 3 employees were attached to the tensioner, requiring support of at least 15,000 pounds, on or about November 11, 2016.
