29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: A) Welfle, Inc., located at 797 State Route 61 East, Norwalk, OH 44857: On or about October 25, 2016, the employer failed to establish a lockout/tagout program for the control of the hazardous energy, which includes procedures, training, and periodic inspections, while performing activities such as, but not limited to, conducting maintenance of the equipment such as, but not limited to, the Roadtec RX500 asphalt milling machine (Model RX500-4-211, Engine Serial # 79384021). Employees were exposed to amputation hazards.
