29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access or, when such an extension was not possible because of the ladder's length, the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail was not provided to assist employees in mounting and dismounting the ladder:  JOBSITE:  The side rails of the Little Giant Ladder used by employees to access the roof of a house did not extend at least 3 feet above the upper landing surface.

JOBSITE:  The employees, using a Little Giant Ladder to access the roof of a house to perform residential roofing work were not trained by a competent person in the proper procedures to minimize the hazards associated with ladders.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   JOBSITE:  The employees, performing residential roofing work on a 4 in 12 pitched roof were not protected from falling between 9 feet 2 inches to 16 feet 3 inches to the ground by the use of guardrails, personal fall arrest system, or other equivalent means.  JM Best Connecticut Roofing LLC was previously cited for a violation of this Occupational Safety and Health standard {29 CFR 1926.501(b)(13)}, which was contained in OSHA Inspection Number 1012123, Citation Number 01, Item Number 002 and was affirmed as a final order on 04/06/2015, with respect to a workplace located at 243 Bartholomew Road, Middletown, CT 06457.
