29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: a. On or about November 3, 2016, in framing deck area, employees working near the waste conveyor which leads to a waste hopper were not protected from a caught-in hazard, in that there was no guarding on three shaft points that were protruding from their respective drums.
