29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  At 20234 Thompson Hall Rd, Fairhope, Alabama:  On or about November 2, 2016 and at times prior; the employer allowed his employee to deck  a high slope roof without fall protection exposing his employee to a fall hazard of 9 feet 4 inches to the ground below.

29 CFR 1926.1053(a)(1)(i): Self supporting portable ladders were not capable of supporting, without failure, at least four times the maximum intended load:  a)  At 20234 Thompson Hall Rd. Fairhope, Alabama:  On or about November 2, 2016 and at times prior; the employer allowed his employees to use a job made ladder to gain access to the roof and the ladder did not have braces installed between the ladder rungs exposing the employee to a fall hazard of 9 feet 4 inches to the ground below.
