29 CFR 1926.20(b)(2): Accident prevention responsibilities: Such programs did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employer: (a) Assurance Contractors at 4177 S. Quebec St., Denver, CO 80237: On and before November 3, 2016, as a part of its accident prevention responsibilities, the employer did not ensure that frequent and regular inspections of the job site, materials, and equipment were made by competent persons designated by the employer. A subcontractor and its employees were working on a two-story single family residence without the use of fall protection. Condition exposed subcontractor employees to fall hazards of up to approximately 20 feet.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems unless another provision in paragraph (b) of this section provides for an alternative fall protection measure: (a) Assurance Contractors, at 4177 S. Quebec St., Denver, CO 80237: On and before November 3, 2016, the controlling, exposing, and correcting employer failed to ensure that an employee and sub-contractor employees performing roofing work on a two-story single family residence were adequately protected against fall hazards. Condition exposed an employee and Assurance Contractors sub-contractor employees to fall hazards of approximately 20 feet.
