29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i). On 10/25/16, at the EVG Autobender control operator station, employees were exposed to 277 v. live electrical parts when accessing the reset button/mechanism for the machine control station located in the same cabinet as live exposed electrical equipment.

29 CFR 1910.147(c)(6)(i)(B): The periodic inspection of the energy control procedure was not conducted to correct any deviations or inadequacies identified. On 10/25/16, at the 3811 Bank Street, Duluth, Georgia facility, the employer did not ensure the periodic inspection of the employer's Lockout/Tagout program corrected for deficiencies in that the review did not correct that Lockout procedures did not include magnitude of the energies listed within the procedures.
