29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. a) Jobsite: Cake processing department, line three, underside of coconut tumbler, left side - An employee cleaning the coconut tumbler machine was exposed to an entanglement and in running nip point hazards from a rotating green disc operating without a guard. The disc was located 1 foot from the left side of the open sided enclosure. Noted on or about 10/24/16.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed. a) Jobsite: Cake processing department, line three, coconut tumbler - An employee cleaning the coconut tumbler machine was exposed to an amputation hazard from a mechanically driven chain operating without a complete enclosure. While cleaning in a crouched position, the employee placed her hand on top of the metal enclosure behind the Arrow Hart on/off switch, which is connected to the enclosure housing the mechanically operated chain, and suffered two finger amputations after coming in contact with the exposed section of chain. b) Jobsite: Cake processing department, line three, underside of coconut tumbler, right side - An employee cleaning the coconut tumbler machine was exposed to an amputation hazard from a mechanically driven chain operating without a complete enclosure. Noted on or about 10/24/16.
