29 CFR 1910.147(c)(2)(ii): The employer's tagout energy control program procedures did not successfully comply with 29 CFR 1910.147(c)(3) and no lockout was provided for employee protection:    On or about 11/3/16 during the poultry processing operations; the "Buddy System" was utilized instead of using the site's established lockout/tagout procedures when performing maintenance/servicing on the cage dumpers; thus exposing employees to amputation/laceration injuries/hazards.
