29 CFR 1910.333(a)(1): The employer failed to ensure that live parts were de-energized before employees began to work on or near live parts. Inside the central utility plant building, the employer did not ensure the employee de-energized the chiller pump and the motor control bucket prior to working on it exposing him to electrical burn hazards.

29 CFR 1910.333(b)(2): While an employee was exposed to contact with parts of fixed electric equipment or circuits which were deenergized, the circuits energizing the parts were not locked out or tagged or both in accordance with the requirements of this paragraph. Inside the central utility plant building, the employer did not ensure the employee performed lockout or tagout on the chiller water pump prior to working on it exposing him to electrical shock hazards.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there are potential electrical hazards shall be provided with, and shall use, electrical protective equipment that is appropriate for the specific parts of the body to be protected and for the work to be performed. The employee did not don electrical protective equipment prior to troubleshooting the motor control bucket exposing him to arc flash burns.
