29 CFR 1910.146(c)(7)(iii): Where a space incorrectly classified by the employer as a permit-required confined space was reclassified as a non-permit confined space, the employer did not document the basis for determining that all hazards in a permit space had been eliminated, through a written certification that contained the date, the location of the space, and the signature of the person making the determination: (a) Building 50, Sterile Processing Service; The STERIS Corporation Reliance 130 Cart and Utensil Washer/Disinfector, classified by the employer as a permit-required confined space, was operated as a non permit-required confined space including entry into the space without documentation.

29 CFR 1910.37(b)(4): Signs were not posted along the exit access indicating the direction of travel to the nearest exit and exit discharge when the direction of travel to the exit or exit discharge was not immediately apparent: (a) Southwest corner of Building 116, Rehabilitation; No exit-related signs were posted in the Mechanical Room No. 110 where several doors were present and view of the door to the corridor was obstructed by a steam handler.
