29 CFR 1926.20(b)(2): Such programs did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers. On or about 11/03/2016, sub-contractor employees were working on the roof of a commercial building located at Highway Z, Wentzville, MO. Lawlor Corporation did not conduct an adequate inspection of the job site, materials, and equipment. Employees were exposed to the following: 1. Subcontractor employees were installing decking on a roof of a commercial building. At the time of the inspection, the foreman was observed attaching decking and was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems.

29 CFR 1926.501(b)(10): "Roofing work on Low-slope roofs." Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. On 10/03/2016, an employee form Lawlor Corporation was installing boards on a roof of a commercial building located at 1850 Highway Z, Wentzville, MO. The employee was working on the roof, which was approximately twenty-two feet above the ground, without the proper use of guardrail systems, safety net systems, or personal fall arrest systems or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.
