29 CFR 1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA From 300 or equivalent), was not completed in detail as required by the regulation. On or about 10/27/16, at the Jefferson facility, the employer did not keep the OSHA 300 log in the detail under Column F and Columns K and L (days away and restricted days) as it was prescribed by the regulation and under the section instead had descriptions like the following: 1 - 5/30/2016 - Lifting table onto pallet 2 - 6/7/2016 - Pulled Nestar down on himself 3 - 8/4/2016 - Lifting boxes onto pallet 4 - 8/30/2016 - States she was pinned between 2 sets of pallets by fork lift driver 5 - 10/3/2016 - Cart she was pushing was hit by pallet jack causing jarring in her left arm 6 - 10/19/2016 - Associate states he climbed into compactor to get his glove

29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to items of this section. On 10/27/16, at the Jefferson facility, the employer has not ensured lockout specific written procedures include magnitudes of all energies involved and indicate specifically the location of disconnects for various electrical equipment.
