29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation:  a)  Building #37:  On or about November 03, 2016, employees were exposed to a fall hazard greater than 10 feet to the dirt ground below, while performing masonry work from a tubular welded frame scaffold without a leg bearing on a base plate and mud sills or adequate firm foundation.  b)  Building #37:  On or about November 03, 2016, employees were exposed to a fall hazard greater than 10 feet to the dirt ground below, while performing masonry work from a tubular welded frame scaffold with a leg bearing on a base plate without mud sills.  c) Building #37:  On or about November 03, 2016, employees were exposed to a fall hazard greater than 10 feet to the dirt ground below, while performing masonry work from a tubular welded frame scaffold with a leg bearing on a base plate without mud sills.

29 CFR 1926.451(c)(2)(i): Footings were not level, sound, rigid, and capable of supporting the loaded scaffold without settling or displacement:  a)  Building #37:  On or about November 03, 2016, employees were exposed to a fall hazard greater than 10 feet to the dirt ground below, while performing masonry work from a tubular welded frame scaffold with a baseplate bearing on unleveled stacked materials such as plywood and concrete blocks.  b)  Building #37:  On or about November 03, 2016, employees were exposed to a fall hazard greater than 10 feet to the dirt ground below, while performing masonry work from a tubular welded frame scaffold with a baseplate bearing partially on a mudsill and unleveled ground.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  Building #37:  On or about November 03, 2016, employees were exposed to a fall greater than 10 feet to the dirt ground below, while accessing the tubular welded frame scaffold without a ladder or other means installed to aid employees in accessing the work platform.
