29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    At the jobsite located near 12800 W 110th Terrace Overland Park KS:  Employees engaged in residential roofing activities from a 4:12 pitch roof were exposed to fall hazards of approximately 10 feet from the eave of the structure to the ground below in that the ladder used to access the roof was not extended at least 3 feet above the eave and was not secured to the structure.

Employees engaged in forklift operation activities at the job site located near 12800 W 110th Terrace Overland Park KS were exposed to struck-by hazards in that forklift training had not been completed to include formal instruction and practical training.
