29 CFR 1926.28(a): The employer did not require the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions:    a)     On or about October 21, 2016, and at times prior, the controlling employer, Dahn Corporation, Inc., did not require subcontracted employees, who were engaged in cement block cutting operations using a chop saw, to utilize appropriate personal protective equipment, such as eye and respiratory protection.  During this operation, an employee was observed exposed to hazards, including but are not limited to, crystalline silica dust and flying cement chips.
