29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. (a) Toledo Mechanical Insulation, Inc. on or about November 07, 2016 at the address of 5836 Southwyck Blvd. Toledo, OH: The employer did not ensure employees installing insulation on duct work atop a commercial type structure were adequately protected from falls. The employees working in this condition were exposed to an approximately 56 ft. fall to the ground below.
