29 CFR 1910.23(a)(7): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:      a) On or about October 19, 2016 and at times prior, a foreman fell approximately 20 feet through the roof of a commercial building. The foreman and an employee were preparing an opening of the roof of approximately 4' wide x 5' long for HVAC equipment to be installed. The roof of the building was installed with multiple anchor systems but the foreman and the employee were not using any fall protection system.
