Single Plate Roofing, LLC, 415 West 12th Street, Erie, PA  16501, roof of the structure, on or about November 2, 2016 and times prior thereto:  Employees were operating electric power tools including, but not limited to, DeWalt DW272 VSR drywall screwdrivers.  The power tools were powered via a Power Stroke portable generator.  The power tools were not being used in conjunction with ground-fault circuit interrupters or an assured equipment grounding conductor program.

Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  A.)   Single Plate Roofing, LLC, 415 West 12th Street, Erie, PA  16501, roof of the structure, on or about November 2, 2016 and times prior thereto:  Employees were performing roofing activities including, but not limited to, installing a stand in seam roof.  Employees who were working outside of the warning line system were not utilizing other means or methods of fall protection, including, but not limited to, guardrail systems, safety net systems, safety monitors and/or personal fall arrest systems.  Employees were exposed to a 19-foot and 20.7-foot fall hazard to the ground below.
