On or about November 8, 2016, in the detached warehouse #650, on the West wall behind the boat a duplex outlet was missing a cover plate, the outlet and wires were protruding from the electrical box thereby exposing employees to electrical and fire hazards. b. On or about November 8, 2016, on the detached warehouse #500, in the entry vestibule a duplex outlet was missing a cover plate thereby exposing employees to electrical and fire hazards.
