29 CFR 1910.178(a)(5): The user did not request, for trucks equipped with front-end attachments other than factory installed attachments, that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered. At the production building, the employer used a Clark Model TMX17 Type F, fork lift equipped with a Global Boom - Model LM-1T-4-11, Serial S1299314 and did not ensure an approved data plate indicating the attachments and the weight and capacity adjustments was affixed or marked on the forklift truck exposing employees to crush-by hazards.
