29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet nor contact any lower level: (a) Easy & Fast Roofing LLC, at 5929 W 16th Street, Denver, CO: On and preceding 11/3/16, the employer did not ensure that personal fall arrest systems, when stopping a fall, were rigged such that an employee could not free fall more than 6 feet nor contact any lower level. An employee of Easy & Fast Roofing LLC installed hardieboard and membrane upon the flat roofs of stairwells upon the roof of the building. The stairwells were approximately 8.5 feet in height. The employee wore a Personal Fall Arrest System (PFAS) consisting of a body harness, connected via a three foot shock absorbing lanyard to a rope grab attached to a lifeline. The lifeline was connected to a tie-off adapter, which was wrapped around a vertical structural member of the stairwell. The anchor point was located beneath the height of the worker. The anchor point was located approximately 5.5 feet above the surface of the building roof. The PFAS was not rigged to prevent contact with the lower level. This condition exposed the employee to an approximate 8.5 foot fall hazard.
