29 CFR 1910.37(a)(3): Materials or equipment were placed, either permanently or temporarily, within the exit route:  In the metal shop, the Tennsmith shear work rest projected 34.5 inches into the exit route. Employees were exposed to fire hazards from having a limited path of egress.

29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):  Energy control procedure(s) did not specifically outline steps for de-energizing the LVD press brake, such as when employees perform die changes. Employees were exposed to amputation and/or crushing hazards when changing dies on the press brake without controlling hazardous energy.

29 CFR 1910.147(c)(7)(i)(A): Each authorized employee had not received training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control:  Training on controlling hazardous energy sources was not provided to employees who perform die changes in the LVD press brake. Employees were exposed to amputation and/or crushing hazards when changing dies in the press brake without locking the equipment out.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:  In the metal shop, a portable fire extinguisher was blocked by a floor fan, oil rag can, and broom, preventing employee access to it. Employees were exposed to fire hazards.

29 CFR 1910.303(g)(1)(i)(B): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the width of the working space in front of electrical equipment was less than the width of the equipment or 762 mm (30 inches), whichever was greater:  In the metal shop, electrical panel(s) located along the wall were blocked by stored items, such as but not limited to corrugated materials, sheet metal, and a sign. This prevents access to the panels for maintenance and/or during emergencies.  Naval Facilities Engineering Command Southwest was previously cited for a violation of this occupational safety and health standard or its equivalent standard [29 CFR 1910.303(g)(1)(i)(B)], which was contained in OSHA inspection number 1123111, citation number 2, item number 2 and was affirmed as a final order on July 25, 2016, with respect to a workplace located at Building 3213, 32nd Street Naval Base San Diego, San Diego, California 92136.
