Employee performing maintenance activities such as, but not limited to greasing bearings on the Superior Telascopic conveyor without fall protection was exposed to a fall hazard of approximately 30 feet.

29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees, in that the employer did not ensure the safety of the equipment pursuant to the considerations in sections (i) through (viii) of 29 CR 1910.303(b)(1):         An orange extension cord plugged into the outside receptacle located at the electrical control panel boxes near the conveyor was energizing the water pump had damage to the outer sheathing exposing employees to electrical shock hazards.

The employer failed to ensure that lockout/tagout procedures were utilized and followed on the Telastacker Conveyor when a crew of employees were performing general maintenance on the conveyor exposing employees to unexpected energization.      The Johnson County Pipe was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(f)(3)(i), which was contained in OSHA inspection number 953849, citation number 1, item number 2b and was affirmed as a final order on July 15, 2014 with a final abatement date of July 17, 2014, with respect to a workplace located at 800 CR 209, Alvarado, TX.

The employer failed to report to OSHA a work-related hospitalization within 24 hours, a hospitalization occurred on 22 October 2016 and was not reported to OSHA until 24 October 2016.
