OR IN THE ALTERNATIVE      Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to hazardous energy:      (a) On or about November 2, 2016 for the employee who sustained an arm amputation where the bottom auger clean-out door was opened on an Unverferth, Brent grain cart, model no.1396, serial no. B31560149 and the auger was powered on at the vicinity of 114th Avenue SE and 88th Street SE Oakes, North Dakota.      Abatement Note: Among other methods, one feasible and acceptable abatement method to correct the hazard would be for the employer to develop and implement an energy control program in accordance with ANSI/ASSE Z244.1  2016, The Control of Hazardous Energy Lockout, Tagout and Alternative Methods and the equipment and machine manufacture manuals.
