29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, was not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system:    location: south tower, 6th floor  on or about: 12/22/16    a) Employees were leaning out of a wall opening while installing temporary support for a concrete pour. The employees, who were exposed to a fall of approximately 50 feet, were not provided with adequate fall protection. The employees were not provided with fall protection.          location: center amenity  on or about: 12/22/16    b) Employees were installing metal studs, 23 1/2 feet above the lower level. The employees were not provided with fall protection.           NOTE: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.        The Extreme Construction Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 1076326, citation number 1, item number 1 and was affirmed as a final order on 8/31/2015, with respect to a workplace located at 23 Westerly Rd.  Ossining, NY.
