EACH 1910 STANDARD USED MUST INCLUDE "AND AS REFERENCED BY THE NOTE CONTAINED IN 1926.602(d)" at the end of the SAVE.    a) Exterior of Building - Driveway: Employer did not certify that the employee engaged in framing activities loading sheets of plywood had been trained and evaluated in the safe operation of an All Terrain Forklift, on or about 12/3/16.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a) Exterior of Building - 2nd Floor: Employees engaged in framing activities were using an extension ladder that only extended approximately 1 foot over the landing surface as means of access to the second floor and were exposed to fall hazards of approximately 10 feet, on or about 12/3/16.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   a) Exterior of Building - 2nd Floor: Employees without any means of fall protection were engaged in framing activities and  exposed to fall hazards of approximately 10 feet, on or about 12/3/16.   Extreme Construction Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 1076325, citation number 01, item number 01 and was affirmed as a final order on 8/31/15, with respect to a workplace located at 23 Westerly Rd., Ossining NY.
