29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii) location: north tower roof below elevator control room overhang on or about: 12/22/16 a) An employee was working from the first level of a tubular welded frame scaffold. The platform was not fully decked. NOTE: Because abatement of this violation is already documented in the inspection casefile, the employer need not submit certification or documentation of abatement for this violation as normally required by 29 CFR 1903.19.

29 CFR 1926.451(f)(14): Makeshift devices, such as but not limited to boxes and barrels, were used on top of scaffold platforms to increase the working level height of employees: location: north tower, front of building on or about: 11/3/16 a) Employees were working from a suspension scaffold that was 100 feet above the ground. The employees were standing on the midrail and sitting on the toprail of the scaffold. NOTE: Because abatement of this violation is already documented in the inspection casefile, the employer need not submit certification or documentation of abatement for this violation as normally required by 29 CFR 1903.19.
