29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a) Jobsite: 2700 Leona Lane, Coraopolis, PA- On or about October 25, 2016, three employees were working as the grounds crew to clean up debris that was being removed from the roof above them. The employees were not provided with head protection to protect them from possible head injury due to the possible impact of falling tools and debris.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  (a) Jobsite: 2700 Leona Lane, Coraopolis, PA- On or about October 25, 2016, grounds crew employees were working to clean up debris and materials from the roofers above and were not provided with eye protection. Employees were exposed to flying particles from cleaning up the debris coming off of the roof above them.

Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  (a) Jobsite- 2700 Leona Lane, Coraopolis, PA: On or about October 25, 2016, an employee removing debris from a low-slope roof that measured approximately 8 feet from ground to eave was not protected from falling to lower levels.
