29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a) Jobsite, Lot 238, Franklin Crossing, Cranberry, PA 16066: On or before November 1, 2016, employees were not trained in the hazards of falling or the proper methods to protect themselves from falling by using fall arrest equipment. Employees were installing shingles on a 12/12 pitched roof exposed to fall hazards between 13 and 30 feet while working at the ends of the roof and at the window.

29 CFR 1926.152(d)(2): A portable fire extinguisher having a rating of not less than 20-B units was not located between 25 feet and 75 feet from flammable liquid storage area(s) located outside: (a) Jobsite, Lot 238, Franklin Crossing, Cranberry, PA 16066: On or before November 1, 2016, employees were using a gasoline powered air compressor and storing gasoline in a five gallon container without having a fire extinguisher available at the jobsite.
