On or about November 8, 2016 three employees were working on the top tier of a four tier welded tubular frame scaffold system. The scaffold system was erected and assembled by the employees, none of which were a competent person defined by the employer to recognize deficiencies and hazards associated with the scaffolds' erection and use.

On or about November 8, 2016 employees working on a 2 story 6:12 pitch steep slope roof were not protected by complete personal fall protection system, guard rails or safety nets, exposing them to falls in excess of 20 feet to the ground below.
