Employees on this site were exposed to hazards including, but not limited to struck by and electrical hazards. Condition noted on or about 09/26/16. NOTE: IN ADDITION TO ABATEMENT DOCUMENTATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
