29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):  a)  At the establishment, on or about 11/10/16:  Lockout/tagout procedures were not adequate in that there were no specific procedural steps for shutting down, isolating, blocking and securing machines and equipment to control all hazardous energy, no specific procedural steps for placement, removal and transfer of lockout devices and no specific procedural requirements for testing a machine or equipment to determine and verify the effectiveness of the lockout devices.  Abatement certification must be submitted for this item.

29 CFR 1910.157(g)(1): An educational program was not provided for all employees to familiarize them with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting:  a)  At the establishment, on or about 11/10/16:  Training was not provided to employees on the use of fire extinguishers and the hazards involved with incipient stage fire fighting.  *ABATEMENT NOTE:  By this date the employer must either correct the alleged violation or implement a Fire Safety Policy as outline in 29 CFR 1910.38 and 29 CFR 1910.39 which includes the evacuation requirements of 29 CFR 1910.157(b).
