29 CFR 1910.178(m)(5)(i): When a powered industrial truck is left unattended, load engaging means shall be fully lowered, controls shall be neutralized, power shall be shut off, and brakes set. Wheels shall be blocked if the truck is parked on an incline. (a) SW Corner of the warehouse - On or about November 9, 2016 an employee was exposed to a fall hazard while riding on an unauthorized wooden pallet used as a scaffold. This pallet was elevated by a Toyota forklift model 8FGU 50 serial no. 10523243. This pallet was not secured to the forklift.

(a) SW Corner of the warehouse - On or about November 9, 2016 an employee was exposed to a fall hazard while riding on an unauthorized wooden pallet used as a scaffold. This pallet was elevated by a Toyota forklift model 8FGU 50 serial no. 10523243. This pallet was not designed by the manufacture to be used as a platform to transport employees.

29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. (a) Shelves Counter inside the warehouse - On or about November 9, 2016 employees were exposed to a flying object hazard while using a metal sledge hammer to install metal shelves. Neither of these employees was wearing eye protection. The employer did not enforce the wearing of safety glasses on this job-site.

(a) Custom Product Corporation - On or about November 9, 2016 the employer did not ensure industrial operator training once every 3 years. The forklift operator has not been re-certified in over 10 years. The forklift driver hoisted an employee on an unauthorized wooden pallet.
