29 CFR 1926.453(b)(2)(v): A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  1) Building 3: 19 Weatherly Street, Atlanta, GA, on or about November 4, 2016, an employee was working inside a Genie aerial lift truck (model number Z60/34 and serial number 1058511) without wearing a personal fall arrest system.  The employer did not enforce the use of fall protection or positioning device attached by lanyard to the basket or the boom of the lift.  The employee was caulking on the second level of the patio deck.
